Incoming Student-Athlete Information

Oberlin College Sports Medicine needs several pieces of information prior to your participation in ANY varsity intercollegiate athletics.

The forms you need are listed below.

Incoming Athlete Information Form (PDF)

ALL incoming student-athletes must complete the Sickle Cell Trait Form. (This form is ONLY needed for varsity sports.)

Any student-athletes who have been diagnosed, and are currently taking medication for, ADD/ADHD, must also complete and submit the ADD/ADHD Medication Form. (This form is ONLY needed for varsity sports.)

Most of the medications prescribed for this type of condition are banned under the NCAA drug testing policy. Prior to your beginning athletic participation, we are required by the NCAA to document the reasons you are taking these banned substances.

Copy of both sides of your medical insurance card. Please enlarge to 150% of its original size.
